Child ADD/ADHD Self-Test

If, regarding your child, you answer six or more of the following questions affirmatively, and the inattention has persisted for at least 6 months to a degree that is maladaptive and inconsistent with developmental level, your child may have ADD or ADHD and should be evaluated by a trained psychological professional for possible treatment.

Does your child often...

  • fail to give close attention to details or makes careless mistakes in schoolwork or other activities?
  • have difficulty sustaining attention in tasks or play activities?
  • have difficulty listening when spoken to directly?
  • show difficulty following through on instruction, often failing to finish schoolwork, chores or duties in the workplace (not due to oppositional behavior or failure to understand instructions)?
  • have difficulty organizing tasks and activities?
  • avoid, dislike or is reluctant to engage in tasks that require sustained mental effort (such as schoolwork or homework.)?
  • lose things necessary for tasks or activities (e.g., toys school assignments, pencils, books, or tools.)?
  • 8. find him or herself easily distracted by extraneous stimuli?
  • show forgetfulness in daily activities?
